LIFE Pittsburgh, Inc. is an organization that assists the older adult in maintaining quality, dignity, and autonomy in their lives by supporting and nurturing the elements of a quality life within the terms of the individual and to support the family unit within the context of a holistic and faith based approach to medical and social services.
About LIFE Pittsburgh:
It is a unique cost-free nursing home alternative program helping frail, older adults meet their daily needs through a full range of available services which includes medical, nursing, hospitalization and long term care.
LIFE Pittsburgh participants benefit from the coordination of all medical appointments,medical transportation, medication management, as well as in-home and adult day health services.
It is our goal to help individuals maintain their own independence through regular medical supervision and social interaction
Eligibility Criteria:
All enrolled participants are
·Eligible for Medicare and at least 55 years old;
·Have limited income and assets;
·Live within the 48 zip codes that make up our service area;

·Have medical conditions that require nursing home level of care;
